Community Health Worker Program

Connecting individuals and families to care, education, and prevention services across our community.

Our Community Health Worker (CHW) Program is a newer service at Loup Basin Public Health Department focused on helping individuals and families connect to the care and resources they need. Community Health Workers provide hands-on support by meeting people where they are, helping navigate health and social services, and reducing barriers to care such as transportation, access, and system complexity.

Our CHW team serves in a variety of specialized roles across the district. One CHW serves as a certified CPR instructor, helping community members gain life-saving skills. Another supports families through our Healthy Families home visiting program, offering guidance and connection during pregnancy and early childhood. Our third CHW leads skin cancer prevention efforts, focusing on education, outreach, and early detection awareness. Together, this team strengthens community health through education, prevention, and direct support.

A Community Health Worker (CHW) is a trained public health professional who helps individuals and families navigate health and social services. CHWs are not doctors or nurses, but they play an important role in bridging the gap between communities and the healthcare system. They provide education, support, and connection to resources, helping people overcome barriers and access the services they need. Their work is rooted in trust, prevention, and making care more accessible for everyone.
